Pre-Participation Physical and Clearance Information
Before student athletes can participate in athletics, they need to have completed all of the necessary documentation. This is done through Home Campus. All of the student-athlete's information is entered there. In addition, they will need to upload a current physical form that is signed by a qualified medical practicioner. If you need a physical form you can download that below.
